Step 1. Submission
Thank you for submitting your online clearance with us.
Step 2: Exit Interview
Fill out the exit interview questionnaire, in order to be cleared with OSA.
Step 3: Wait for completed clearance
Once the clearance has been completely signed, you will be notified via email.
Step 4: Request official documents
You may proceed to request for your transcript of records, diploma, and other certifications here.
When claiming your requested document, you will be asked to submit any ONE of the following:
- School ID
- Affidavit of Loss
- Affidavit of Non-Issuance of ID
