Learning Delivery Modes
Congratulations on your admission to UP Cebu. This guide will help you through your enrolment process, including document submission, health clearance, tuition options, and key deadlines.
STEP 1: Review Eligibility and Prepare for Enrolment

- You must have a final offer of admission to a degree program in UP Cebu.
- Request for UPG (University Predicted Grade) will not be entertained.
- You must read and accept the Privacy Notice for Students.
- Prepare requirements as listed below:
All physical copies must be prepared and arranged exactly in the order listed upon submission to the OUR.

5 | FORM 137 GRADE 7-12
(Official Secondary Permanent Records). Complete from Junior High School (JHS) to Senior High School (SHS) Grade 7-12.
Important: If your JHS and SHS is the same: JHS-SHS Form 137 Grade 7-12 must be original, printed and complete to the letter.
If JHS and SHS is different: JHS Form 137 Grade 7-10 must be stamped as a certified true copy and attached to the Original printed SHS Form 137 Grade 11-12. SHS Form 137 Grade 11-12 must not be stamped as a Certified true copy. |
Direct-to-school option
Use this computer-generated Form 137 Letter Request for your school to release it to you. Read the letter and ensure the school complies to the expected remarks.
The school can release the document to the student as the letter request authorizes the bearer to hand-carry the document in a sealed envelope. The student is not allowed to open the sealed envelope.
Additional notarized Affidavit of Non-Enrolment and Gap Year is required if students did not enrol in college succeeding their last enrolment from SHS.
For schools who prefer to send Form 137 directly, this is acceptable. Students just need to present proof of transaction or certification from school that the document will be submitted this way and preferably with the expected date of submission. |

*Note: Entrance Credentials are permanent records of the University of the Philippines Cebu and may no longer be borrowed for whatever purpose after submission to the Office of the University Registrar. Secure photocopies (where applicable) for future needs.
AASS and Varsity Program entrants must secure at least two original copies of their PSA Birth Certificate—one to be submitted to the Office of the University Registrar (OUR), and one personal copy for use in future participation in sports activities or events.
Talent Test result, if you applied for the Fine Arts Program.

STEP 2: Submit Online Requirements
You will receive an email from the Office of the University Registrar with a personalized access code and link to the UP Cebu Student Registration Form. Use this to upload requirements 1, 2, 3, 6, 7, and 9.
You will be asked to choose your preferred PE and NSTP schedule in the same form.
STEP 3: Medical and Dental Requirements

- Medical Examination. This requires laboratory tests, like:

- Chest X-ray
- CBC
- You may go to the laboratory examinations at our affiliated laboratory: Alpha Diagnostic Center, Corner General, F. Ramos St., Echavez Ext, Cebu City, 6000 Cebu, (032) 254 7305. You may also have your laboratory examination in a laboratory of your choice.
- Just secure a request form from the UP Cebu Clinic.
- Keep a photocopy of your laboratory results. You cannot borrow these results when submitted to the UP Cebu Clinic. Place the original documents together with an ID picture inside a long brown envelope for submission.

- Dental Examination.

- Those who have dental problems are advised to seek treatment from a dentist before visiting the school clinic.

- Visit the UP Cebu Clinic with results. Clinic hours are from Mon-Fri, 8:00–11:00AM & 1:00–3:00PM
- Bring the results to the UP Cebu Clinic where you will fill out a form that the UP Cebu physician will duly sign.
- After completing your medical and dental examinations, take your medical certificate for submission to the Office of the University Registrar.
*Students under 18 years old must complete the Data Privacy Consent Form signed by a parent or guardian. This is required to process your Admission Slip.

STEP 4: Submit Physical Documents to the OUR
After submitting the online form and completing medical/dental clearance, hand-carry the following to the Office of the University Registrar (OUR:

- Printed copies of the online requirements (items, 1, 2, 3, 4, 5, 6, 7 and 8)
- Health clearance (from Step 3)

The OUR will issue an Admission Slip after verification.
You will then proceed to your College Secretary.
STEP 5: Tuition Options (RA 10931)

- Email to the OUR ([email protected]) your duly signed Voluntary Opt-out Form, if applicable, and ask to be assessed.
- Pay your tuition and other miscellaneous school fees through Philippine Veterans Bank (21-0000-067) via online channels such as gcash, instapay, pesonet, and bank transfers.
[If paying via GCash, please add 4 zeroes at the start to have 13 digits, 000021-0000-067.]
Make sure your payment status is COMPLETED or SUCCESSFUL.
Email your proof of payment (must include amount paid, transaction date, time, and reference number).
For GCash transactions, make sure to enable mobile or email notifications. Attach proof of completed payment from GCash SMS or GCash Email with InstaPay Trace Number. (also found in your GCash Inbox, “Your Bank Transfer Status” message)
Incomplete information may lead to delays or the non-processing of your enrollment as your payment could not be verified.

STEP 6: College Enrollment and Official Registration

- Proceed to your College Secretary with your Admission Slip.
- Your courses will be encoded, and you will be issued a Form 5A (Temporary Form 5).
- Paying students must pay tuition fees.
- Upon confirmation of payment, the College Secretary will issue your Form 5.
- You are now officially enrolled.
YOUR ACCEPTANCE TO THE UNIVERSITY OF THE PHILIPPINES CEBU IS PROVISIONAL SUBJECT TO THE VERIFICATION AND SUBMISSION OF ORIGINAL COPIES OF THE REQUIREMENTS (3, 4, 5, & 6) LISTED ABOVE.

STEP 7: Scholarship and Dorminatory (if applicable)
If you have or are looking for a Scholarship, please contact OSA.
If you want to apply for the Dormitory, please fill out the Dormitory Application Form. Online application is temporarily suspended. The application process for university dormitories will reopen once the enrollment period for the First Semester of Academic Year 2025-2026 begins.
Note: Please hold off on submitting your application until you have finished enrolling and obtained a copy of your Form 5 (or Temporary Form 5 if available) for the First Semester of Academic Year 2025-2026. Also, kindly ensure to provide the 2024 Income Tax Returns for both of your parents. We will only accept applications with all requirements fully completed.

Notes:
Universal Access to Quality Tertiary Education Act of 2017
UP Cebu shall implement Republic Act 10931 known as the Universal Access to Quality Tertiary Education Act of 2017 and its Implementing Rules and Regulations (IRR) that was approved and signed on 2018 February 22.
Anent to this are the Implementation Guidelines for the Grant of Free Tuition and Other School Fees in UP dated 2019 September 3.
Opting Out of Free Tuition
UP Cebu students may also voluntarily opt out from availing the Free Tuition privilege. Should the student voluntarily opt out, the student will pay the full tuition and other fees, as his/her contribution to the University.
Please refer to the Implementation Guidelines for the Grant of Free Tuition and Other School Fees in UP (pages 12-15) for the steps in the Opt-Out process.
